On the day of your initial visit, bring your identification and arrive a little early to complete the necessary forms.

Reminders & Required Items

To ensure a smooth and efficient first visit, please review the following guidelines before coming to the
clinic and prepare the necessary documents to help your doctor perform a complete assessment.

Please bring the following on the day of your appointment:

Medical documents that help explain your history Examples: referral letters, previous examination reports, summaries from other clinics or hospitals
Eye drops or related supplements If you are currently using any, please bring them for your doctor’s reference
Allergy medication list or long-term prescriptions This helps determine whether your condition may be related to ongoing treatments

Avoid wearing makeup Especially eyeshadow and mascara, to allow accurate evaluation of the ocular surface
Make an appointment in advance Our clinic operates by appointment; please arrive on time for your scheduled slot
